To schedule an initial consultation, use the registration button below, or you can call our office. We've implemented an online process to facilitate registration. We are a small group practice that values privacy, confidentiality, and personalized care. We do not use administrative staff for the purposes of scheduling, managing correspondence, or registration. REGISTRATION
To book an initial consultation, select the registration button below and complete the form. You’ll get an email confirming we’ve received it. In around one to two business days, we’ll create a secure patient portal account for you and send a link to your email with instructions to complete this setup. The secure portal is where you can schedule appointments, complete paperwork, have access to your information, and message your doctor.
Our online process is secure and encrypted to protect your privacy and personal health information.
If you run into any issues online and need to speak with someone, please call our office, we may also call you if we have further questions during this process. Completing these initial steps does not establish you as a patient under our care, this is established with your physician. See assessment process below for an details.
ASSESSMENT PROCESS
The assessment phase may last between two to four visits. These first sessions are dedicated to understanding you and the problems that bring you to treatment as comprehensively and deeply as possible. Our goal is to develop a relationship in which we are mutually, respectfully, and thoughtfully engaged in understanding the problems at hand. We will then discuss your preliminary diagnosis and we will develop a treatment plan together. From there, decisions about the type, frequency, and duration of treatment follow. We will explore multiple different treatment options with you, which might include a medical workup, medications, psychotherapy, neuromodulation, nutritional and lifestyle changes and/or a combination of these. Together we will weigh the risks and benefits of each option and select the treatment plan that feels right for you. At this point, once we are in agreement with your treatment plan, we will mutually agree to enter a physician-patient relationship. Should we feel that based on our assessment, your needs may be better met by another facility or program, we will provide you with referrals to appropriate care
